เรติคูลาร์ฟอร์เมชั่น (อังกฤษ: reticular formation; ตัวย่อ RF) เป็นกลุ่มนิวเคลียสประสาทที่เชื่อมต่อกันรวม ๆ เป็นส่วนที่มีกายวิภาคไม่ชัดเจนเพราะอยู่กระจายไปทั่วก้านสมอง เซลล์ประสาทของ RF ประกอบเป็นเครือข่ายประสาทที่ซับซ้อนภายในแกนของก้านสมองที่อยู่กระจายเริ่มตั้งแต่ส่วนบนของสมองส่วนกลาง ต่อไปถึงพอนส์ จนไปถึงส่วนล่างของก้านสมองส่วนท้าย[1] RF รวมเอาวิถีประสาทที่ส่งขึ้นไปยังเปลือกสมอง เป็น ascending reticular activating system (ARAS) และวิถีประสาทที่ลงไปยังไขสันหลังคือ reticulospinal tracts[2][3][4][5]

เซลล์ประสาทของ RF โดยเฉพาะที่เป็นส่วนของ ARAS มีบทบาทสำคัญยิ่งในการดำรงรักษาความตื่นตัวทางพฤติกรรมและความรู้สึกตัว (consciousness) หน้าที่ของ RF รวมทั้งการปรับควบคุม (modulatory) และหน้าที่ทาง premotor[A] หน้าที่โดยละเอียดรวมทั้งการควบคุมการสั่งการกล้ามเนื้อ, การควบคุมหัวใจและหลอดเลือด, การลดความเจ็บ, การหลับกับการตื่น และการปรับให้เคยชิน[7] เซลล์ประสาทด้านบน (rostral) โดยหลักมีหน้าที่ปรับควบคุม และเซลล์ประสาทด้านล่าง (caudal) โดยมากมีหน้าที่ทาง premotor

RF แบ่งออกเป็น 3 แถวในแนวตั้ง (คือคอลัมน์)[8]

  • raphe nuclei (แนวกลางคือ median) เป็นแหล่งสังเคราะห์และหลั่งเซโรโทนินไปทั่วสมอง เซโรโทนินมีบทบาทควบคุมอารมณ์ที่สำคัญ
  • gigantocellular reticular nuclei (แนวด้านในคือ medial) มีบทบาทประสานการเคลื่อนไหว (motor coordination) เป็นต้น
  • parvocellular reticular nuclei (แนวด้านข้างคือ lateral) มีหน้าที่เกี่ยวกับการหายใจออกเป็นต้น

RF ขาดไม่ได้เพื่อควบคุมการทำงานพื้นฐานของสิ่งมีชีวิตระดับสูง เป็นส่วนที่เก่าแก่ที่สุดในสมองตามประวัติวิวัฒนาการชาติพันธุ์

โครงสร้างทั่วไป[แก้]

RF ในมนุษย์ประกอบด้วยนิวเคลียสประสาทเกือบร้อยอัน ซึ่งส่งเส้นประสาทไปยังสมองส่วนต่าง ๆ มากมายรวมทั้งสมองส่วนหน้า ก้านสมอง และสมองน้อยเป็นต้น[2] องค์ประกอบต่าง ๆ รวมทั้ง reticular nuclei[B], ใยประสาทที่ส่งไปยังทาลามัส (reticulothalamic projection fiber) แล้วส่งต่อเป็นใยประสาทจากทาลามัสไปทั่วเปลือกสมอง (thalamocortical projection), ใยประสาทที่ใช้สารสื่อประสาท acetylcholine (ACh) และส่งไปยังสมองส่วนที่สูงขึ้น (เป็น ascending cholinergic projections), ใยประสาทที่ไม่ได้ใช้สารสื่อประสาท ACh และส่งไปยังระบบประสาทที่ต่ำกว่า (descending non-cholinergic projections) และใยประสาทที่ส่งไปยังไขสันหลัง (reticulospinal projections)[3] RF ยังมีระบบประสาทย่อยหลัก ๆ สองอย่าง คือระบบ ascending reticular activating system (ARAS) ซึ่งส่งกระแสประสาทไปยังเปลือกสมอง และลำเส้นใยประสาท reticulospinal tracts ซึ่งส่งไปยังไขสันหลัง ทั้งสองช่วยอำนวยกระบวนการทางประชานและทางสรีรภาพต่าง ๆ[2][3] มีการแบ่งส่วนตามหน้าที่ตามทั้งระนาบซ้ายขวา (sagittal) และระนาบหน้าหลัง (coronal)

โดยทั่วไป reticular nuclei จะแบ่งออกเป็น 3 คอลัมน์ คือ

  • คอลัมน์กลาง (median column) คือ raphe nuclei
  • คอลัมน์ใน (medial column) คือ gigantocellular reticular nuclei (ชื่อระบุว่าเซลล์มีขนาดใหญ่กว่า)
  • คอลัมน์ข้าง (lateral column) คือ parvocellular nuclei (ชื่อระบุว่าเซลล์มีขนาดเล็กกว่า)

ส่วนการแบ่งการทำงานดั้งเดิมเป็นแบบบน-ล่าง คือแบบทางจมูก-หาง (rostral และ caudal) เพราะได้สังเกตว่า การทำรอยโรคที่ส่วนทางจมูก (rostral) ของแมวทำให้มีอาการนอนมาก (hypersomnia) เทียบกับการทำรอยโรคที่ส่วนทางหาง (caudal) ทำให้แมวนอนไม่หลับ (insomnia) จึงเกิดแนวคิดว่า ส่วนทางหาง/ด้านล่างยับยั้งการทำงานส่วนทางจมูก/ด้านบน

แต่การแบ่งตามระนาบซ้ายขวา (sagittal) ก็แสดงความต่างทางสัณฐานได้ดีกว่า คือ raphe nuclei เป็นสันตั้งขึ้นอยู่ตรงกลางของ RF โดยมีส่วนต่อจากมันที่เรียกว่า medial reticular formation (medial RF) ซึ่งมีขนาดใหญ่และมีวิถีประสาททั้งส่งขึ้นและลงที่ยาว และล้อมรอบด้วย lateral reticular formation (lateral RF) ซึ่งอยู่ใกล้ ๆ กับนิวเคลียสประสาทสั่งการ (motor nuclei) ของประสาทสมอง และมีหน้าที่อำนวยการทำงานของนิวเคลียสประสาทสั่งการ

medial RF และ lateral RF [แก้]

medial RF และ lateral RF เป็นนิวเคลียสประสาทในสองคอลัมน์ มีขอบเขตไม่ชัดเจน และส่งกระแสประสาทผ่านเมดัลลาเข้าไปในสมองส่วนกลาง อาจแยกนิวเคลียสเหล่านี้ได้โดยหน้าที่ ประเภทเซลล์ และเส้นใยประสาทนำเข้า (afferent) และนำออก (efferent) ถ้าตามลงล่าง (caudal) จากสมองส่วนกลางส่วนบน (rostral) ไปที่จุดระหว่างสมองส่วนกลางกับพอนส์ส่วนบน (rostral) ส่วน medial RF จะเห็นชัดน้อยลง แต่ lateral RF จะเห็นชัดขึ้น[9]

ข้าง ๆ ทั้งสองของ medial RF เป็น lateral RF ซึ่งเห็นชัดเป็นพิเศษที่ระหว่างพอนส์ส่วนล่าง (caudal) กับเมดัลลาส่วนบน (rostral) บริเวณนี้เป็นแหล่งเกิดของประสาทสมองรวมทั้งเส้นประสาทเวกัส (CN X) เป็นต้น ปมประสาทของ lateral RF และอินเตอร์นิวรอนของมันรอบ ๆ ประสาทสมอง ก่อรีเฟล็กซ์โดยเฉพาะ ๆ ของมันและอำนวยให้ทำหน้าที่อื่น ๆ ของมันได้

หน้าที่ทั่วไป[แก้]

RF มีเครือข่ายประสาทมากกว่า 100 เครือข่าย โดยมีหน้าที่ต่าง ๆ รวมทั้ง

  1. การควบคุมการสั่งการกล้ามเนื้อ (somatic motor control) คือ เซลล์ประสาทสั่งการบนบางส่วนส่งแอกซอนไปยัง RF ซึ่งกลายเป็นลำเส้นใยประสาท reticulospinal tract ที่ส่งไปยังไขสันหลัง มีหน้าที่ดำรงความตึงกล้ามเนื้อ ดุลร่างกาย และท่าทาง[C] โดยเฉพาะเมื่อเคลื่อนไหวร่างกาย RF ยังส่งข้อมูลทางตาและหูไปยังสมองน้อยซึ่งรวบรวมข้อมูลทางตา หู และการทรงตัวเพื่อประสานการเคลื่อนไหวของร่างกาย นิวเคลียสประสาทสั่งการ (motor nuclei) อื่น ๆ รวมทั้งศูนย์ทอดสายตา (gaze center) ซึ่งทำให้ตาสามารถตามและตรึงวัตถุที่มองได้ และ central pattern generator (CPG)[D] ซึ่งสร้างกระแสประสาทเป็นคาบ ๆ สำหรับการหายใจและการกลืน
  2. การควบคุมหัวใจและหลอดเลือด คือที่ระดับก้านสมองส่วนท้าย RF มีศูนย์ควบคุมหัวใจ (cardiac) ซึ่งควบคุมอัตราและกำลังการเต้นของหัวใจ และศูนย์ปรับขนาดหลอดเลือด (vasomotor) ซึ่งปรับขยายหลอดเลือด เป็นการเปลี่ยนความดันและการไหลเวียนของเลือด
  3. การลดความเจ็บ คือ RF เป็นวิถีประสาทวิถีหนึ่งที่ข้อมูลความเจ็บปวดจากร่างกายส่วนล่างผ่านขึ้นไปถึงเปลือกสมอง และเป็นแหล่งกำเนิดของวิถีประสาทลงล่างที่ระงับความเจ็บปวด (descending analgesic pathways) ซึ่งทำการที่ไขสันหลังไม่ให้ส่งสัญญาณความเจ็บปวดบางส่วนไปยังสมอง
  4. การหลับและความรู้สึกตัว คือ RF ส่งกระแสประสาทไปยังทาลามัสและเปลือกสมอง ซึ่งทำให้สามารถควบคุมได้บ้างว่าข้อมูลประสาทสัมผัสอะไรบ้างจะส่งไปถึงสมองใหญ่ อันเป็นข้อมูลที่เรารู้สึกและใส่ใจได้ จึงมีบทบาทสำคัญในสภาวะเกี่ยวกับความรู้สึกตัวเช่น ความตื่นตัว (alertness) และการหลับ การบาดเจ็บที่ RC อาจมีผลเป็นสภาพโคม่าอย่างแก้ไม่ได้
  5. การปรับให้เคยชิน (habituation) เป็นกระบวนการที่สมองเรียนรู้เพื่อไม่สนใจสิ่งเร้าที่เกิดซ้ำ ๆ แต่ไร้ประโยชน์ ในขณะที่ยังคงไวต่อสิ่งเร้าอื่น ๆ เช่น บุคคลอาจหลับนอนได้ในเมืองใหญ่ที่การจราจรเสียงดัง แต่สามารถลุกขึ้นอย่างทันใดเพราะนาฬิกาปลุกหรือทารกร้องไห้ นิวเคลียสประสาทของ RF ซึ่งควบคุมระดับการทำงานของเปลือกสมองเป็นส่วนของ ascending reticular activating system[13][7]

ระบบย่อยหลัก ๆ[แก้]

Ascending reticular activating system (ARAS)[แก้]

ascending reticular activating system (ตัวย่อ ARAS) หรือเรียกอีกอย่างว่า extrathalamic control modulatory system หรือ reticular activating system (ตัวย่อ RAS) เป็นกลุ่มนิวเคลียสประสาทที่เชื่อมต่อกันในสมองสัตว์มีกระดูกสันหลัง มีหน้าที่ควบคุมความตื่น (wakefulness) และการตื่นหลับ (sleep-wake transition) เป็นส่วนของ reticular formation (RF) โดยมากประกอบด้วยนิวเคลียสต่าง ๆ ในทาลามัสและนิวเคลียสที่ใช้สารสื่อประสาทโดพามีน (dopaminergic), สาร norepinephrine (noradrenergic), สารเซโรโทนิน (serotonergic), สารฮิสตามีน (histaminergic), สาร acetylcholine (cholinergic) และสารกลูตาเมต (glutamatergic)[E][2][14][15][16]

โครงสร้างของ ARAS[แก้]

ARAS ประกอบด้วยวงจรประสาท/เครือข่ายประสาทหลายหน่วยซึ่งเชื่อมส่วนหลัง (dorsal) ของสมองส่วนกลางด้านหลัง (posterior midbrain) และพอนส์ส่วนหน้า (anterior) กับเปลือกสมองผ่านวิถีประสาทโดยเฉพาะ ๆ ที่วิ่งผ่านทาลามัสและไฮโปทาลามัส[2][15][16] ARAS เป็นกลุ่มนิวเคลียสโดยมีมากกว่า 20 หน่วยในแต่ละข้างของก้านสมองส่วนบน พอนส์ เมดัลลา และไฮโปทาลามัสส่วนหลัง สารสื่อประสาทที่เซลล์ประสาทเหล่านี้หลั่งรวมทั้งโดพามีน, norepinephrine, เซโรโทนิน, ฮิสตามีน, acetylcholine และกลูตาเมต[2][14][15][16] ARAS มีอิทธิพลต่อเปลือกสมองด้วยแอกซอนที่ส่งไปโดยตรงและโดยอ้อมด้วยแอกซอนที่ส่งผ่านทาลามัส[15][16][17]

วิถีประสาทผ่านทาลามัสโดยหลักประกอบด้วยเซลล์ประสาทที่ใช้สารสื่อประสาท acetylcholine ใน pontine tegmentum เทียบกับวิถีประสาทผ่านไฮโปทาลามัสที่โดยหลักประกอบด้วยเซลล์ที่หลั่งสารสื่อประสาทกลุ่ม monoamine ซึ่งก็คือ โดพามีน, norepinephrine, serotonin และฮิสตามีน[2][14] ส่วนเซลล์ประสาทที่หลั่งกลูตาเมตใน ARAS พึ่งระบุได้ไม่นานเทียบกับเซลล์ประสาทแบบ monoaminergic และ cholinergic[E][18] เซลล์ประสาทที่หลั่งกลูตาเมตรวมนิวเคลียสหน่วยหนึ่งในไฮโปทาลามัสและนิวเคลียสต่าง ๆ ในก้านสมอง[15][18][19] เซลล์ประสาทที่หลั่งนิวโรเพปไทด์ (neuropeptide) คือ orexin ภายในไฮโปทาลามัสส่วนข้าง (lateral hypothalamus) ส่งแอกซอนไปยังองค์ประกอบทุกส่วนของ ARAS และมีหน้าที่ประสานงานของระบบ[16][20][21]

ARAS ประกอบด้วยส่วนสมองที่เก่าแก่ตามประวัติวิวัฒนาการ ซึ่งขาดไม่ได้เพื่อการอยู่รอด จึงยังทำงานอยู่แม้เมื่อสัตว์ตัวแข็ง (คือใต้ภาวะ animal hypnosis[F])[26] ส่วนที่ส่งแอกซอนเพื่อควบคุมประสาทไปยังเปลือกสมองโดยมากเชื่อมกับ prefrontal cortex[27] แต่ดูเหมือนจะเชื่อมต่อกับเขตต่าง ๆ ของระบบประสาทสั่งการในเปลือกสมองไม่มาก[27]

หน้าที่ของ ARAS[แก้]

ความรู้สึกตัว[แก้]

ARAS เป็นปัจจัยสำคัญให้รู้สึกตัว (consciousness)[17] มีบทบาทต่อความตื่น (wakefulness) ซึ่งกำหนดโดยความตื่นตัว (arousal) ของเปลือกสมองและความตื่นตัวทางพฤติกรรม[5]

การควบคุมการหลับ-ตื่น[แก้]

หน้าที่หลักของ ARAS ก็คือเปลี่ยนและเพิ่มการทำงานของทาลามัสและเปลือกสมอง จนกระทั่งคลื่นไฟฟ้าสมอง (EEG) ไม่ประสาน (desynchronized)[G][29][30] การทำงานทางไฟฟ้าในสมองจะต่างกันเมื่อตื่นและหลับ คลื่นสมองที่ความต่างศักย์ต่ำและเปลี่ยนแปลงเร็ว (fast burst) ซึ่งเป็นสภาวะที่เรียกว่า EEG desynchronization (อีอีจีไม่ประสาน) สัมพันธ์กับความตื่นและการนอนหลับระยะ REM คือสภาพทั้งสองมีการทำงานทางไฟฟ้าสรีรภาพคล้าย ๆ กัน ส่วนคลื่นสมองที่ความต่างศักย์สูงและเปลี่ยนแปลงช้า (slow wave) จะพบในระยะนอนหลับนอกเหนือจาก REM เมื่อเซลล์ประสาทรีเลย์ในทาลามัสส่งกระแสประสาทแบบ burst (คือส่งในอัตราสูงพักหนึ่งแล้วพัก) EEG โดยทั่วไปแล้วอยู่ในสภาวะ synchronized (ประสาน) แต่ถ้าส่งกระแสประสาทแบบ tonic (คือส่งเรื่อย ๆ) EEG ก็อยู่ในสภาวะ desynchronized (ไม่ประสาน)[30] การกระตุ้น ARAS จะมีผลให้อีอีจีไม่ประสานโดยระงับคลื่นเปลือกสมองที่ช้า (0.3-1 เฮิรตซ์), คลื่นเดลตา (1-4 เฮิรตซ์) และ spindle wave oscillation (11-14 เฮิรตซ์) และโดยสนับสนุนคลื่นแกมมา (20-40 เฮิรตซ์)[20]

สภาพทางสรีระที่เปลี่ยนจากการหลับลึกไปเป็นตื่นสามารถผันกลับไปมาได้โดยมี ARAS เป็นตัวอำนวย[31] ไฮโปทาลามัสส่วน ventrolateral preoptic nucleus (VLPO) มีหน้าที่ยับยั้งวงจรประสาทที่ก่อสภาวะตื่น ดังนั้น เมื่อ VLPO ทำงาน ก็จะทำให้เริ่มสภาวะการหลับ[32] เมื่อนอนหลับอยู่ เซลล์ประสาทใน ARAS มีอัตรากระแสประสาทที่ต่ำกว่ามาก และในนัยตรงกันข้าม ก็จะส่งกระแสประสาทในอัตราที่สูงกว่าเมื่อตื่น[33] เพื่อให้สมองหลับได้ จึงต้องลดการทำงานของ ARAS เพื่อลดการส่งกระแสประสาทขึ้นไปยังเปลือกสมอง[31]

ความใส่ใจ[แก้]

ARAS ยังช่วยอำนวยให้เปลี่ยนสรีรภาพจากความตื่นแบบสบาย ๆ ไปเป็นแบบใส่ใจสูง[24] เลือดที่ไหลไปในบริเวณจะมากขึ้น (ซึ่งสมมุติว่า บ่งการทำงานของเซลล์ประสาทที่เพิ่มขึ้น) ใน midbrain reticular formation และ thalamic intralaminar nuclei ในช่วงทำกิจที่ต้องตื่นตัวมากต้องใส่ใจมาก

ความสำคัญทางคลินิกของ ARAS[แก้]

การมีรอยโรคขนาดใหญ่ที่นิวเคลียสของ ARAS ในก้านสมองอาจมีผลเปลี่ยนแปลงระดับความรู้สึกตัวอย่างรุนแรง (เช่น โคม่า)[34] RF ในระดับสมองส่วนกลางที่เสียหายทั้งสองด้านอาจก่อสภาวะโคม่าจนถึงตาย[35]

การกระตุ้น ARAS โดยตรงด้วยไฟฟ้าทำให้แมวตอบสนองแบบเจ็บและทำให้มนุษย์รายงานว่าเจ็บ[ต้องการอ้างอิง] การทำงานของ ARAS ในแมวอาจทำให้รูม่านตาขยาย[ต้องการอ้างอิง] ซึ่งอาจเป็นเพราะเจ็บนาน ผลเหล่านี้บ่งว่า วงจรประสาทของ ARAS สัมพันธ์กับวิถีประสาทของความเจ็บปวดทางสรีรภาพ[36]

โรค[แก้]

โรคบางอย่างเกี่ยวกับ ARAS อาจเกิดตามอายุ เพราะ ARAS ดูเหมือนจะตอบสนองน้อยลงโดยทั่วไปเมื่ออายุมากขึ้น[37] ความเปลี่ยนแปลงของความคู่ควบกันทางไฟฟ้า (electrical coupling)[H] เสนอว่า สามารถอธิบายความเปลี่ยนแปลงการทำงานของ ARAS โดยบางส่วน คือ ถ้าความคู่ควบกันลดลง การทำงานประสานกันแบบความถี่สูง (gamma band) ก็จะลดลงตาม ในนัยตรงกันข้าม ถ้าความคู่ควบกันเพิ่มขึ้น การทำงานประสานกันแบบความถี่สูงก็จะเพิ่มขึ้น ซึ่งอาจเพิ่มความตื่นตัวหรือหรือก่อการหลับระยะ REM[39]

โดยเฉพาะแล้ว การขัดการทำงานของ ARAS ยกว่าเป็นเหตุของโรคต่าง ๆ ต่อไปนี้

  • ภาวะง่วงเกิน - รอยโรคที่ pedunculopontine tegmental nucleus (PPT/PPN) / laterodorsal tegmental nucleus (LDT) สัมพันธ์กับภาวะนี้[40] กระแสประสาทที่ส่งออกจาก PPN ที่ลดลง และการหลั่งเพปไทด์ orexin ที่เสียไป จะทำให้ง่วงผิดปกติตอนกลางวันซึ่งเป็นอาการของโรค[20]
  • อัมพาตแบบ progressive supranuclear palsy (PSP) - มีการเสนอว่า การทำงานผิดปกติของการส่งสัญญาณด้วยไนตรัสออกไซด์เป็นปัจจัยให้เกิด PSP[41]

ปัจจัยทางพัฒนาการ[แก้]

มีปัจจัยหลายอย่างที่อาจมีผลลบต่อพัฒนาการของ ARAS

  • การคลอดก่อนกำหนด[42] - ไม่ว่าจะหนักเท่าไรหรืออยู่ในครรภ์มากี่อาทิตย์ การคลอดก่อนกำหนดก่อผลลบที่คงยืนตลอดระยะพัฒนาการต่อกระบวนการ pre-attention คือความตื่นตัวและการตื่นกับการนอน ต่อการใส่ใจ (attention) คือความเร็วการเกิดปฏิกิริยาต่อความรู้สึกทางประสาทสัมผัสและการกรองข้อมูลทางประสาทสัมผัส และต่อกลไกในเปลือกสมอง

Descending reticulospinal tracts (RST)[แก้]

ลำเส้นใยประสาทในไขสันหลัง reticulospinal tract มีป้ายเป็นสีแดง ทางซ้ายตรงกลางในรูป

reticulospinal tract (RST) หรือเรียกอีกอย่างว่า anterior reticulospinal tract เป็นลำเส้นใยประสาทของระบบประสาทสั่งการนอกพีระมิด (extrapyramidal system) ที่ส่งลงไปจาก reticular formation (RF[44] โดยส่งเป็นสองลำไปที่เซลล์ประสาทสั่งการซึ่งส่งเส้นประสาทไปยังกล้ามเนื้อยืด (extensor) และกล้ามเนื้องอ (flexor) ของลำตัวและต้นแขนขา และโดยหลักมีหน้าที่เกี่ยวกับการเคลื่อนที่ (locomotion) และการควบคุมท่าทาง (posture control)[C][10] แต่ก็ยังมีหน้าที่อื่น ๆ อีกด้วย รวมทั้งเป็นวิถีประสาทลงล่างที่ระงับความเจ็บปวด (descending analgesic pathways) ปรับกล้ามเนื้อเพื่อรักษาความตึงกล้ามเนื้อและการทรงตัวร่างกาย[7] และหน้าที่ทางการหายใจ[45]

RST เป็นวิถีประสาทหลักวิถีหนึ่งใน 4 วิถี ซึ่งส่งจากเปลือกสมองไปที่ไขสันหลังเพื่อควบคุมการทำงานของกล้ามเนื้อโครงร่าง โดยทำงานร่วมกับวิถีประสาททั้งสามอื่น ๆ เพื่อประสานการเคลื่อนไหวต่าง ๆ รวมทั้งการจัดการวัตถุต่าง ๆ ได้อย่างแยบยล[44] วิถีประสาททั้ง 4 ทางสามารถจัดเป็น 2 ระบบ คือ ระบบด้านใน (medial system) และระบบด้านข้าง (lateral system) ระบบด้านในรวม reticulospinal tract และ vestibulospinal tract โดยทั้งสองมีหน้าที่ควบคุมท่าทาง ส่วน corticospinal tract และ rubrospinal tract อยู่ในระบบด้านข้าง มีหน้าที่ควบคุมการเคลื่อนไหวที่ละเอียด[44]

องค์ประกอบของ reticulospinal tract[แก้]

RST มีองค์ประกอบสองอย่าง ทั้งสองส่งกระแสประสาทไปยังเซลล์ประสาทสั่งการของกล้ามเนื้อผ่านอินเตอร์นิวรอนที่ใช้ร่วมกับลำเส้นใยประสาท corticospinal tract มีหน้าที่เกี่ยวกับการเคลื่อนไหวสองอย่าง คือ การเคลื่อนที่ (locomotion) และการควบคุมท่าทาง (posture control)[C][10][46][47]

  • medial reticulospinal tract มีกำเนิดที่ RF ในพอนส์ ส่งเส้นประสาทไปยังร่างกายซีกเดียวกัน (ipsilateral) ผ่าน anterior funiculus ของไขสันหลัง มีฤทธิ์กระตุ้นกล้ามเนื้อยืดของลำตัว (axial) และของแขนขาส่วนต้น (proximal) เป็นการเพิ่มความตึงของกล้ามเนื้อ และมีฤทธิ์ยับยั้งกล้ามเนื้องอของแขนขาส่วนต้น มีหน้าที่ควบคุมการเคลื่อนไหวของศีรษะและร่างกาย
  • lateral reticulospinal tract มีกำเนิดที่ RF ในก้านสมองส่วนท้าย ส่งเส้นประสาทไปยังซีกร่างกายทั้งสองข้าง (bilateral) ผ่าน lateral funiculus ของไขสันหลัง มีฤทธิ์กระตุ้นกล้ามเนื้องอของแขนขาส่วนต้น มีฤทธิ์ยับยั้งกล้ามเนื้อยืดของลำตัวและของแขนขาส่วนต้น เป็นการลดความตึงของกล้ามเนื้อ มีหน้าที่เกี่ยวกับ righting reflex[I] ของศีรษะและร่างกายเพื่อตอบสนองเมื่อมีตำแหน่งที่ไม่สมดุล

ในทิศทางตรงกันข้าม ลำเส้นใยประสาทที่ส่งข้อมูลประสาทสัมผัสขึ้นไปยังสมองเรียกว่า spinoreticular tract

หน้าที่ของ RST[แก้]

  1. รวบรวมข้อมูลจากระบบประสาทสั่งการเพื่อประสานการเคลื่อนไหวอัตโนมัติเพื่อเคลื่อนที่หรือรักษาท่าทาง
  2. กระตุ้นและยับยั้งการเคลื่อนไหวที่อยู่ใต้อำนาจจิตใจ มีอิทธิพลต่อความตึงกล้ามเนื้อ[48][7]
  3. อำนวยการทำงานของระบบประสาทอิสระ
  4. ควบคุมการส่งข้อมูลประสาทสัมผัสที่ทำให้เจ็บไปยังระบบประสาทกลาง[7]
  5. มีอิทธิพลต่อการไหลของเลือดไปที่ lateral geniculate nucleus ของทาลามัส

ความสำคัญทางคลินิกของ RST[แก้]

ระบบเส้นประสาทที่ส่งจากก้านสมองและสมองน้อยลงไปที่ไขสันหลัง 2 ระบบหลักสามารถทำให้ร่างกายปรับท่าทางโดยอัตโนมัติเพื่อรักษาการทรงตัว (balance) และทิศทางของส่วนต่าง ๆ ของร่างกาย (orientation) คือ ลำเส้นใยประสาท vestibulospinal tract จาก vestibular nuclei (นิวเคลียสหนึ่งของประสาทหู) และ RST จากพอนส์และเมดัลลา รอยโรคในลำเส้นใยประสาทเหล่านี้ก่อภาวะกล้ามเนื้อเสียสหการ (ataxia) และความไม่เสถียรของท่าทาง[49]

ความเสียหายไม่ว่าจะทางกายภาพหรือทางหลอดเลือดต่อก้านสมองที่แยก red nucleus ของสมองส่วนกลางจาก vestibular nuclei ในพอนส์อาจก่อสภาพแข็งเกร็งเสมือนไร้ก้านสมอง (decrebrate rigidity) ที่มีอาการทางประสาทเป็นกล้ามเนื้อที่ตึงขึ้นและ stretch reflex ที่ไวเกิน เมื่อตอบสนองต่อสิ่งเร้าที่ทำให้ตกใจหรือให้เจ็บ ทั้งแขนขาจะยืดออกแล้วหมุน (เช่น หมุนคว่ำฝ่ามือ) เหตุก็คือกระแสประสาทที่ส่งอย่างต่อเนื่องของลำเส้นใยประสาท lateral vestibulospinal tract และ RST ซึ่งกระตุ้นเซลล์ประสาทสั่งการของกล้ามเนื้อเหยียดโดยไม่ได้รับกระแสประสาทยับยั้งจากลำเส้นใยประสาท rubrospinal tract ที่ถูกแยกออก[50]

ความเสียหายที่ก้านสมองเหนือระดับ red nucleus อาจก่อสภาพแข็งเกร็งเสมือนไร้สมองใหญ่ (decorticate rigidity) เมื่อตอบสนองต่อสิ่งเร้าที่ทำให้ตกใจหรือให้เจ็บ แขนจะงอเข้าแต่ขาจะยืดออก เหตุก็คือ red nucleus โดยผ่านลำเส้นใยประสาท rubrospinal tract จะต่อต้านการกระตุ้นเซลล์ประสาทสั่งการของกล้ามเนื้อเหยียดจาก lateral vestibulospinal tract และ RST แต่เพราะ rubrospinal tract ส่งไปถึงไขสันหลังระดับคอเท่านั้น ก็จึงมีผลต่อแขนคือกระตุ้นกล้ามเนื้องอและยับยั้งกล้ามเนื้อยืด แต่ไม่มีผลต่อขา[50]

ความเสียหายต่อก้านสมองส่วนท้ายใต้ระดับ vestibular nuclei อาจก่ออัมพาตอ่อนเปียก (flaccid paralysis) ภาวะกล้ามเนื้อตึงตัวน้อย (hypotonia) การเสียการกระตุ้นเพื่อหายใจ และอัมพาตแขนขาสองข้าง (quadriplegia) คนไข้จะไร้รีเฟล็กซ์เหมือนกับระยะต้น ๆ ของสภาพช็อกเหตุไขสันหลัง (spinal shock) เพราะเซลล์ประสาทสั่งการไม่ทำงานอย่างสิ้นเชิง เหตุก็คือไม่มีกระแสประสาทที่ส่งอย่างต่อเนื่องจาก lateral vestibulospinal tract และ RST อีกต่อไป[50]

ประวัติ[แก้]

นักประสาทกายวิภาคชาวเยอรมัน (Otto Deiters) ได้บัญญัติคำว่า "reticular formation" ในปลายคริสต์ศตวรรษที่ 19 ราว ๆ เวลาเดียวกันกับการตั้งหลักเซลล์ประสาท (neuron doctrine) ของซานเตียโก รามอน อี กาฆัล คำว่า "reticulum" หมายความว่า "โครงสร้างคล้ายตาข่าย" ซึ่ง RF ดูเหมือนเมื่อตรวจดูในยุคต้น ๆ มีการระบุไว้ว่า มันซับซ้อนเกินไปที่จะศึกษา หรือว่าเป็นส่วนสมองที่เหมือนกัน ๆ โดยไม่มีระเบียบอะไรเลย นักประสาทวิทยาศาสตร์ชาวออเสตรีย-อเมริกันผู้ได้รับรางวัลโนเบลสาขาสรีรวิทยาหรือการแพทย์ อีริก แคนเดิล ได้กล่าวว่า มันจัดระเบียบคล้ายกับ intermediate gray matter (ชั้นที่ 7 และ 9) ในไขสันหลัง เพราะสับสนวุ่นวายและละเอียดพิศดารเช่นนี้ จึงทำให้นักวิจัยไม่สนใจตรวจดูเขตสมองนี้[ต้องการอ้างอิง] แม้เซลล์จะไร้เขตแดนเหมือนกับปมประสาท แต่ก็มีการทำงานและประเภทของเซลล์โดยเฉพาะ ๆ ดังนั้น คำว่า "reticular formation" จึงมักใช้เป็นคำกล่าวกว้าง ๆ โดยนักวิทยาศาสตร์อาจระบุนิวเคลียสประสาทโดยเฉพาะ ๆ ที่เป็นส่วนของ RF[ต้องการอ้างอิง]

การกระตุ้นสมองด้วยกระแสไฟฟ้าโดยตรงจะทำให้เซลล์รีเลย์ส่งกระแสประสาทต่อไปยังเปลือกสมอง Magoun ได้ใช้หลักนี้เพื่อแสดงว่า ในก้านสมองของแมว สามารถกระตุ้นส่วนสองส่วนในสมองเพื่อปลุกให้ตื่นเมื่อหลับอยู่ ส่วนแรกคือ วิถีประสาทสัมผัสจากกายและวิถีประสาทจากหูที่ส่งไปยังสมอง ส่วนที่สองคือ ลำดับ "รีเลย์ส่งขึ้นจาก reticular formation ในก้านสมองส่วนล่างผ่าน midbrain tegmentum, subthalamus และไฮโปทาลามัส ไปยัง internal capsule"[51] ส่วนที่สองนี่น่าสนใจมาก เพราะมันไม่เข้ากับวิถีทางกายวิภาคใดที่รู้จักในยุคนั้นซึ่งทำให้ตื่นได้ จึงได้บัญญัติชื่อว่า ascending reticular activating system (ARAS)

ต่อมา ความสำคัญของระบบรีเลย์ที่ค้นพบใหม่นี้ก็ได้ตรวจสอบโดยทำรอยโรคที่สมองส่วนกลางข้างหน้าที่ด้านใน (medial) และด้านข้าง (lateral) แมวที่มีสมองส่วนกลางซึ่ง ARAS เสียหายจะหลับลึกโดยมีคลื่นสมองที่สมกัน แต่เมื่อทำรอยโรคที่วิถีประสาทสัมผัสจากกายและวิถีประสาทจากหู แมวก็ยังมีการตื่นการหลับที่ปกติ และสามารถปลุกให้ตื่นได้ด้วยสัมผัสทางกาย เพราะสิ่งเร้าภายนอกเช่นนี้ย่อมไม่ถึงเปลือกสมองตามปกติ นี่จึงบ่งว่า วิถีประสาทของสัมผัสทางกายต้องส่งขึ้นผ่าน ARAS ด้วย

ท้ายสุด Magoun ได้บันทึกศักย์ไฟฟ้าภายในส่วนใน (medial) ของก้านสมองแล้วพบว่า สิ่งเร้าทางหูส่งเข้าไปยังบางส่วนของ ARAS โดยตรง อนึ่ง การกระตุ้นโดยการช็อกเส้นประสาทไซแอติก (sciatic nerve) ครั้งเดียว ยังเริ่มการทำงานของ medial reticular formation, ไฮโปทาลามัส และทาลามัสอีกด้วย การทำงานของ ARAS ก็ไม่ได้อาศัยการกระจายสัญญาณต่อไปผ่านวงจรประสาทในสมองน้อย เพราะก็ยังได้ผลเดียวกันเมื่อตัดขาดจากสมองน้อย (decerebration) และตัดขาดจากสมองใหญ่ (decortication) นักวิจัยจึงได้เสนอว่า มีคอลัมน์เซลล์รอบ ๆ RF ในสมองส่วนกลางที่ได้กระแสประสาทนำเข้าจากลำเส้นใยประสาทส่งขึ้นทั้งหมดของก้านสมอง แล้วส่งต่อกระแสประสาทเหล่านี้ต่อไปยังเปลือกสมอง และดังนั้น จึงสามารถควบคุมความตื่นได้.[51][31]

It receives information from sensory receptors of various modalities, transmitted through spinoreticular pathways and cranial nerves (trigeminal nerve – polymodal pathways, olfactory nerve, optic nerve and vestibulocochlear nerve – monomodal pathways). These pathways reach the thalamus directly or indirectly via the medial column of reticular formation nuclei (magnocellular nuclei and reticular nuclei of pontine tegmentum). The reticular activating system begins in the dorsal part of the posterior midbrain and anterior pons, continues into the diencephalon, and then divides into two parts reaching the thalamus and hypothalamus, which then project into the cerebral cortex (Fig. 1). The thalamic projection is dominated by cholinergic neurons originating from the pedunculopontine tegmental nucleus of pons and midbrain (PPT) and laterodorsal tegmental nucleus of pons and midbrain (LDT) nuclei [17, 18]. The hypothalamic projection involves noradrenergic neurons of the locus coeruleus (LC) and serotoninergic neurons of the dorsal and median raphe nuclei (DR), which pass through the lateral hypothalamus and reach axons of the histaminergic tubero-mamillary nucleus (TMN), together forming a pathway extending into the forebrain, cortex and hippocampus. Cortical arousal also takes advantage of dopaminergic neurons of the substantia nigra (SN), ventral tegmenti area (VTA) and the periaqueductal grey area (PAG). Fewer cholinergic neurons of the pons and midbrain send projections to the forebrain along the ventral pathway, bypassing the thalamus [19, 20].
