การสับเปลี่ยนของโครโมโซม (chromosomal translocation) เป็นความผิดปกติของโครโมโซมอย่างหนึ่ง เกิดจากการแลกเปลี่ยนชิ้นส่วนของโครโมโซมที่ไม่ใช่คู่กัน จุดที่โครโมโซมมาต่อกันนั้นเกิดการผสมยีนเกิดเป็นยีนใหม่ ซึ่งเป็นสาเหตุที่พบบ่อยอย่างหนึ่งของการเกิดมะเร็ง ความผิดปกตินี้อาจตรวจพบได้จากการตรวจทางเซลล์พันธุศาสตร์หรือการตรวจแครีโอไทป์ของเซลล์ที่มีการสับเปลี่ยน

การสับเปลี่ยนของโครโมโซมแบ่งออกเป็นสองชนิดหลัก คือแบบมีการแลกเปลี่ยน (reciprocal, หรือ "แบบไม่ใช่โรเบิร์ตโซเนียน") และแบบโรเบิร์ตโซเนียน (Rebertsonian) หรืออาจแบ่งเป็น แบบสมดุล (ไม่มีการเพิ่มหรือลดของสารพันธุกรรม และทางทฤษฎีแล้วจะไม่มีการเปลี่ยนแปลงในเชิงหน้าที่เกิดขึ้น) และแบบไม่สมดุล (ซึ่งมีการเพิ่มหรือลดของสารพันธุกรรม ทำให้มียีนเกินขึ้นมาหรือขาดหายไป)

การเขียนชื่อของโครโมโซมที่มีการแลกเปลี่ยนชิ้นส่วนกันโดยทั่วไปยุดตามระบบ The INternational System for Human Cytogenetic Nomenclature (ISCN) โดย t(A;B)(p1;q2) หมายความว่า มีการแลกเปลี่ยนกันของชิ้นส่วนระหว่างโครโมโซม A และ B บางครั้งจะมีการเพิ่มข้อมูลในวงเล็บที่สองด้วยหมายถึงตำแหน่งในโครโมโซมของโครโมโซม A และ B ตามลำดับ โดย p หมายถึงแขนข้างสั้นของโครโมโซม ส่วน q หมายถึงแขนข้างยาว และตัวเลขตามหลัง p หรือ q หมายถึงลำดับของตำแหน่ง (region) แถบ และแถบย่อยของโครโมโซมเมื่อย้อมด้วยสีย้อม
